innovacionThe PACT project intends to design, implement and evaluate a new model of care and intervention for social inclusion provided by the Social Services of Castilla y León.

The model will be based on a new organisational culture and a new model of care, analysing and testing, by means of the pilots, the results and outcomes obtained on the target population and on the new model of care.


1. The Network

The development of a public-private partnership (NETWORK) for the intervention in situations of social exclusion by increasing the efficiency and quality of the system, and unifying the information, methods, resources and knowledge available to the public and private actors in their territory. This will require:

  • The formalization of partnerships between public and private actors in the field of social services and other relevant systems to ensure the most efficient use of the resources and the adoption of shared protocolsconnect-20333_640.
  • The development of consensual comprehensive care processes.
  • Computer management application shared among professionals in the public and private organisations on the Network.

2. Case handling, a new model of social intervention for inclusion

Stepping towards greater levels of inclusion and decreasing the reliance on social services through a new care model for people in exclusion or at risk of exclusion, based on person-centred care. This will require:

  • 13423955_10153699304852817_7966548839056791644_nThe design and implementation of a common multidimensional diagnostic tool for an inclusion-exclusion centred intervention.
  • The training, motivation and engagement of professionals with the new care model.
  • The use of personal experiences and an appreciative approach that focuses on their capacities rather than, as usual, their deficiencies in order to provide care that takes into account their overall situation and their life cycle.
  • The promotion of interventions aimed at the empowerment of people through training and strategies for the development of autonomy, establishing the sequence of impacts and outcomes to be achieved in a personalized project under the supervision, control and monitoring of the case handler.

3. Segmentation

Proactive intervention through the creation and implementation of a population segmentation tool that uses the existing data in the management system to:

  • 1Calculate the risk of emergence of chronic personal and family situations through the determination of critical factors present in each particular circumstance (employability, time spent in care, resilience, vital statistics, cohabitation, etc.)
  • Create alerts on personal situations that may require proactive and/or intensive intervention to prevent further problems or the emergence of chronic dependence.
  • Generate lists of individuals that could potentially participate in community actions.
