Expected outcomes

The expected outcomes are:

A NEW POPULATION STRATEGY that generates a new model of detection and proactive treatment in segmentation

A NEW ORGANISATIONAL CULTURE through the Network in order to achieve an efficient intervention.

A NEW MODEL OF COMPREHENSIVE ATTENTION, with unique access, flexible, adapted and lead by the case handler.

The expected results in the organisation and in the care model for social inclusion are:

– Resources and services are better dimensioned to respond to the demands of citizens, adapted to the nature of the increased needs and the intensity of demand.

– Public and private institutions use the same evaluation standards.

– New software module, integrated within the unified access to social services information system (SAUSS), which allows and guarantees:

  • Procedures related to benefits, resources, programs and services aimed at combating poverty and social exclusion are integrated
  • Reception and unified access to the Social Services System (from any device).
  • The actors share information within the same management system.

– Professionals from entities belonging to the Network know and share the advantages of working in an interconnected system.

– Social inclusion teams apply the case handler methodology.

– People who receive care from inclusion teams have a case handler and an individual care plan agreed between them and the actor involved.

– There are manuals and guidelines available to professionals

– There are training guides for trainers and technicians

– Professionals will have a software solution for the analysis of data from the population receiving care within an area which will provide them:

  • Lists for community-based care.
  • Lists of alerts in situations requiring proactive individual attention, as well as review and prioritization of monitoring.
  • Integration of information regarding different existing or planned intervention, avoiding duplication of activities among actors in the system.
  • Statistics regarding profiles and social risk situations by areas.
  • Control system for the monitoring of the actions and objectives.

New computer tools will not only allow a better use of the data, a better quality in the model of care, an easier access to the system and a faster and full answer for people’s needs, but it will also imply saving in assistance times and management spending for the entities belonging to the Network.

The project results will be used to reassess the social policy of Castilla y León in the fight against poverty and social exclusion as well as to target, define, modify and implement, in its global dimension, the new model, within the scope of the Network. Moreover, the results will allow the acquisition of a deeper and more precise knowledge on the situations of social exclusion: their causes, needs, the people involved, the advantages and strengths of the new model, the weaknesses that must be corrected and the methodology to apply. All this information will be incorporated to the public social services system in Castilla y León.
