PACT in a nutshell
ProActive Case-based Targeted model (PACT) is an initiative aimed at innovation in social services with the objective of designing and testing a new model of care for people at risk of social exclusion in Castilla y León, which will simultaneously combine two interconnected strategies: the public-private partnership and a proactive social investment.
After the design and preparation of the organisational, intervention and population segmentation tools required in this new care model, they will be fully tested simultaneously in four areas of the region with the experimental actions (piloting). The target population will be individuals receiving the Guaranteed Minimum Income Benefit subsidy.
The pilots will be evaluated in depth, in order to generate evidence based on the implementation in real contexts of changes in our approach to social inclusion so key elements can be decided in order to generalize them to the social service system.
PACT will involve the collection and systematization of vital data relating to the impacts and outcomes of the pilots, in order to derive knowledge on the new care model and its implementation throughout the Social Services System in Castilla y Leon, and its dissemination in other national and international contexts.
